Medicare Advantage PPO plans

Preferred provider organization (PPO) plans let you choose any provider who accepts Medicare. You don't need a referral from a primary care provider for specialist or hospital visits. However, if you use a provider in the plan's network, covered services may cost less.

 

Most plans cover added benefits like:

 

  • Fitness benefit
  • Dental, vision and hearing coverage
  • Home-delivered meals after a qualifying inpatient hospital, inpatient psychiatric hospital or skilled nursing facility stay
  • Over-the-counter (OTC) benefit

Chronic Condition Special Needs Plans (C-SNPs)

A Chronic Condition Special Needs Plan (C-SNP) is a Medicare Advantage plan with Part D prescription drug coverage. This plan is for people who have a chronic health condition, like heart disease or diabetes.

 

C-SNPs give you a personal care team, low medical and prescription copays and an Allina Health | Aetna Medicare Extra Benefits Card to help pay for certain everyday expenses.
